Children and youth in foster care have complex health care needs and require careful care coordination to ensure that their needs are met. In order to gain insights towards improvement of the health care system and care coordination for children and youth in foster care, Children Now engaged stakeholders in identifying promising practices in LA County and elevating provider-level, county-level, and statewide program and policy changes that practitioners, policymakers, and those involved in the child welfare system can pursue to improve the health care system and care coordination for children and youth in foster care.
